PHILIPPINE PASSPORT
This is the first time I got my Passport. I attempted many times when I was in Manila and in Cebu but no luck due to time pressure. You need to file a leave for at least 1 to 2 days to complete the process. I went to DFA Tacloban, and I finished the process for only 15 minutes (With power interruption for 3 hours).
For my advice, before you go to the location, you need to prepare first your Documentary Requirements.
DFA Tacloban Fees
For 20 working days = Php 1,200
For 35 working days = Php 950
*If you want to have it delivered in your home address, just pay an additional LBC fee for door-to-door delivery.
For New Applications
"Please have all your original documents and IDs photocopied before going to DFA on your appointment date."
GENERAL REQUIREMENTS
1. Personal appearance
2. Confirmed appointment (For NCR) (But for DFA Tacloban, you must be there at least 1 hour before the office hour; 8AM-5PM)
1. Birth Certificate (BC) in Security Paper (SECPA) issued by the National
Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable. (Report of Birth duly authenticated by NSO if born abroad)
3. No need to submit a passport size photo (They will give the shot using their digital camera)
4. Valid picture IDs and supporting documents to prove identity

- Digitized SSS ID
- Driver’s License
- GSIS E-card
- PRC ID
- IBP ID
- OWWA ID

- Other acceptable picture IDs such as the following:
- Old College ID
- Alumni ID
- Old Employment IDs
LIST OF SUPPORTING DOCUMENTS (Old documents issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant, At least 2 of the following):
- PSA Marriage Contract
- Land Title
- Seaman’s Book
- Elementary or High School Form 137 or Transcript of Records with readable - dry seal
- Government Service Record
- NBI Clearance
- Police Clearance
- Barangay Clearance
- Digitized Postal ID
- Readable SSS-E1 Form or Microfilmed Copy of SSS-E1 Form
- Voter's Certification, List of Voters and Voter's Registration Record (please attach receipt)
- School Yearbook

5. NSO Birth Certificate
1. Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO depending on the NSO signatory.
In Case of No Birth Record
If born in or after 1950 (January 1, 1950 or after):
1. Apply for the delayed registration of birth at the local civil registry office located at the place of birth of applicant
2. Submit authenticated Birth Certificate from NSO and supporting public document/s upon the discretion of the processor or Consular Officer, with correct date and place of birth (i.e. Form 137, Voter’s Registration Record, Baptismal Certificate with readable dry seal or National Commission on Muslim Filipinos (NCMF) / Office on Muslim Affairs (OMA) Certificate with photo and readable dry seal for Muslim applicants).
If born before 1950 and below (December 31, 1949 and below):
1. Certificate of Non-Availability of Record from NSO
2. Notarized Joint Birth Affidavit of Two Disinterested Persons
3. Any public document/s with correct full name, date and place of birth (i.e.
Baptismal Certificate with readable dry seal or NCMF / OMA Certificate with photo and readable dry seal for Muslim applicants)
For Married Women:
1. Marriage Contract (MC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or original and photocopy of the Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Marriage Contract from the LCR is required when entries in NSO Marriage Contract are blurred or unreadable.
2. Report of Marriage duly authenticated by NSO if married abroad
For spouse of foreign national:
1. Marriage Contract (MC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Marriage Contract from the LCR is required when entries in NSO Marriage Contract are blurred or unreadable.
2. Report of Marriage duly authenticated by NSO if married abroad
3. Original and photocopy of Commission of Filipino Overseas (CFO) Guidance and Counseling Certificate of Attendance (required for first time applicants or renewal of passport to be used for the first time the surname of the husband)
Aliens who acquired Filipino citizenship or applicants with foreign-sounding family names (e.g. LIM, CHO, SY, LEE, SAMADI, SINGH, etc.):
1. Bureau of Immigration Identification Certificate
2. Certificate of Naturalization
3. Election of Filipino Citizenship / Oath of Allegiance
4. If necessary,
5. Marriage Certificate of parents (duly authenticated by NSO)
6. NSO Birth Certificate of applicant’s brother or sister
Applicants who availed of Dual Citizenship or Election of Philippine Citizenship under RA 9225:
1. Identification Certificate
2. Oath of Allegiance
3. Order of Approval
4. Report of Birth (for those born abroad)
For Minor Applicants (below 18 years old)
General Requirements:
1. Confirmed appointment (except for 1 year old and below)
2. Personal appearance of the minor applicant
3. Personal appearance of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child)
4. Original Birth Certificate of minor in Security Paper issued by NSO or Certified True Copy of Birth Certificate issued by the Local Civil Registrar and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable. Report of Birth duly authenticated by NSO is required if minor was born abroad.
5. Document of identity with photo, if minor is 8-17 years old (for first time and renewal applicant) such as School ID or Form 137 with readable dry seal
-For minor applicants who never attended school, a Notarized Affidavit of Explanation executed by either parent (if minor is a legitimate child) / by mother (if minor is an illegitimate child) detailing the reasons why the child is not in school, is required
6. Marriage Certificate of minor’s parents duly authenticated by NSO
7. Notarized Affidavit of Support and Consent to travel from either parent (if minor is a legitimate child) / from mother (if minor is an illegitimate child)
8. Original and photocopy of valid passport of the person traveling with the minor
9. Photocopy of valid passport of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child) or identification documents (Please refer to List of Acceptable IDs above)
Additional Requirements:
If minor is not traveling with either parent or alone
1. Personal appearance of the mother is required if minor applicant is an illegitimate child.
2. Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the traveling companion and relationship to the minor. If minor will be traveling alone, ASC must be executed by either parent, stating that his/her child will be traveling alone. If minor is illegitimate, mother should execute the ASC.
3. Original and photocopy of DSWD Clearance
-There is no need to secure a DSWD Clearance if the minor traveling abroad has parents who are in the Foreign Service or living abroad or are immigrants, provided he / she is holding a valid pass such as a dependent’s visa / pass / identification card or permanent resident visa / pass / identification card which serves as proof that he / she is living with parents abroad.
If both parents are abroad:
1. Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the traveling companion (authenticated by the nearest Philippine Embassy or Consulate General). If minor is illegitimate, mother should execute the ASC.
2. Special Power of Attorney (SPA) with an attached photocopy of either parent’s valid passport (authenticated by the nearest Philippine Embassy or Consulate General) authorizing a representative in assisting the child to apply for a passport. If minor is illegitimate, mother should execute the SPA.
3. Original and photocopy of DSWD Clearance
-There is no need to secure a DSWD Clearance if the minor traveling abroad has parents who are in the Foreign Service or living abroad or are immigrants, provided he / she is holding a valid pass such as a dependent’s visa / pass / identification card or permanent resident visa / pass / identification card which serves as proof that he / she is living with parents abroad.
4. Proper ID of the duly authorized representative (Please refer to List of Acceptable IDs)
If minor is legitimated by subsequent marriage of parents:
1. Authenticated Birth Certificate from NSO must include annotation regarding new status as legitimated and the full name of the child
If minor is illegitimate but acknowledged by father:
1. Birth Certificate from NSO reflecting surname of father with Affidavit of Acknowledgement and Consent to use the surname of father
Foundling:
What is Foundling?
An infant that has been abandoned by its parents and is discovered and cared for by others.
1. Certificate of foundling authenticated by NSO
2. DSWD Clearance
3. Passport of the person who found the applicant
4. Letter of authority or endorsement from DSWD for the issuance of passport
Orphaned minor applicant:
1. Authenticated Death Certificates of parents from NSO
2. Court order awarding guardianship of the orphaned minor applicant or substitute parental authority under Article 214 & 216 of the Family Code
3. DSWD Clearance
Abandoned minor applicant:
1. Court order awarding guardianship of the abandoned minor applicant or substitute parental authority
2. DSWD Travel Clearance
Legally adopted:
1. Original and Certified True Copy (CTC) of NSO Birth Certificate before adoption
2. Original and Certified True Copy (CTC) of the NSO amended Birth Certificate after adoption
3. Certified True Copy (CTC) of the Court Decision or Order on Adoption and Certificate of Finality
4. DSWD clearance for minor applicant, if traveling with the person other than the adopting parents
In case the applicant is for adoption by foreign parents:
1. Certified True Copy of the Court Decree of Abandonment of Child
2. NSO Death Certificate of the child’s parents or the Deed of Voluntary Commitment executed after the birth of the child
3. Endorsement of child to the Intercountry Adoption Board by the DSWD
4. Authenticated Birth or Foundling Certificate
Minor applicant whose parents are annulled / divorced:
1. Court order awarding guardianship of the minor applicant or substitute parental authority
2. DSWD Travel Clearance
3. NSO Marriage Certificate with annotation on nullity or annulment decree
Minor applicant whose mother is likewise a minor:
1. Personal appearance of mother and maternal grandparent/s
2. NSO Birth Certificate of minor applicant and mother
3. Affidavit of Support and Consent executed by the maternal grandparent/s indicating the name of the traveling companion
4. DSWD Clearance if minor will be traveling with the person other than the maternal grandparent/s
5. Proof of identity of mother and maternal grandparent/s (Please refer to List of Acceptable IDs above)
For Muslim Applicants (same general requirements stated above)
For late registered Muslim applicants:
1. Certificate of Tribal Affiliation from the National Commission on Muslim Filipinos (NCMF)
For converts who would like to use their Muslim name:
1. Annotated Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) bearing the Muslim name
2. National Commission on Muslim Filipinos (NCMF) or Office on Muslim Affairs (OMA) Certificate of Conversion
If applicant is a DOST Scholar (same general requirements stated above):
1. Secure a certification or clearance from DOST

DeleteWalang online ang pagkuha ng passport in Tacloban. All you have to do is to be there early in the morning. Magdala ka ng 2 valid IDs, yung government issued IDs. Tapos orginal na birth certificate. Ipakita mo ang requirements mo sa guard, fill up ka ng form, and submit sa counter. Wait ka for your name to be called. Kasi mag seset kayo ng picture taking para na sa passport but magbabayad ka muna. May expedite at ang regular time frame. Nandun sa loob ang LBC, kung gusto mo imail sa bahay niyo, may additional payment lang sa LBC. Dala ka kahit mga 2K lahat ng hindi ka mashort.
Tanung LNG po,ilang araw bago makuha ang passport for renewal po.dn no need mag pa set ng appointment sa dfa tac?plz. Reply

Regular Processing – P950 – 15 working days
Rush Processing – P 1200 – 7 working days
Kung gusto mo ipadala sa inyo directly, may table sila for LBC, I'm not so sure if magkano na ibabayad, basta prepare ka mga P100 to 200 for the special mailing.

DeleteNaku, I'm sorry, I don't think pwede. Strict kasi ang passporting sa ngayon. Infact, yung birth certificate mo dapat in security paper and original para makuha talaga nila ang exact detail mo. And sa ID kasi, kailangan yung government issued talaga, make sure to secure nalang baka pauwiin lang friend mo.
